Chronic non-treated posterior fracture-dislocation of the shoulder

Posterior fracture-dislocations often remain undiagnosed at initial medical attendance. In dislocation, the head of humerus extends beyond glenoid to form a zone impaction, which “fixes” it. The injury is almost unidentifiable in standard frontal X-ray images. Meanwhile, continued fixation state posterior dislocation leads rapid progression traumatic impaction over up 50% articular surface area. associated damage lip scapula, rupture rotator cuff muscles, symptoms shoulder instability after relocation, and severe pain syndrome require advanced treatments for this type injury. Here we report clinical case anatomical neck fracture with displaced consolidation, combined dislocation. To avoid subacromial impingement, instead correcting position head, abandoned reposition performed an osteotomy distal displacement greater tubercle humerus.
